Discussion of Transition Issues with Families,
and Support for Families in the Transition Process
(FACTS/LRE grants permission for photocopying of this handout.)

Agreements must specify when and how each of these will take place and who is
responsible:
- Acquainting families with procedures, issues, legal rights
- Helping families identify the roles they want to play and issues that are important to
them
- Amending IFSPs to include transition plans
- Helping families become familiar with (spectrum of) programs for which children may be
eligible
- Providing support to families in preparing their child(ren) for the transition
- Helping families identify or develop options for care and education in the event their
children are not found eligible for special education, and for extended hours of care even
when they are expected to be eligible
